Those who served in the U.S. military may be eligible for health benefits through the Veterans Administration (VA). Generally for those who served after 1979, the VA requires at least 24 continuous months of service. However, there are a number of exceptions to the minimum duty requirements, including disability in the line of duty. Any person discharged under dishonorable conditions is not eligible for benefits. An individual can apply for health benefits through the VA using the internet, in person, by telephone, or by mail. To apply online, complete and submit the form found at https://www.1010ez.med.va.gov/. No additional documents or signature is required. To apply in person simply go to your local VA health care facility to fill out and submit the form. You can also apply by telephone by calling 1-877-222-VETS (8387). Representatives are available Monday-Friday from 8 am to 8 pm. When applying by telephone, a VA representative will fill out the application with you. The completed application will be mailed to your address for your signature. Once signed you are required to mail the application to Health Eligibility Center, 2957 Clairmont Road, Suite 200, Atlanta, GA 30329-1647. To apply by mail you can download the application online or pick up the application from your local VA health care facility, fill out, and submit to Health Eligibility Center, 2957 Clairmont Road, Suite 200, Atlanta, GA 30329-1647. The signature of the applicant or the person appointed as Power of Attorney will be required when applying in person, by phone, or by mail. If the appointed Power of Attorney is signing the application, a copy of the Power of Attorney will also be required.
