At Tully Law Group, our Elder Care Coordinators are dedicated to enhancing the lives of the seniors within our community. An Elder Care Coordinator (ECC) is a professional, such as a social worker, counselor, nurse, or gerontologist, who assists older people and their families to attain the highest quality of life given their circumstances. This past week, our ECC team visited a series of senior care facilities on Long Island. The goal? To establish lasting relationships and share our services. Please have a look at some of the places we had the opportunity to visit:

Raina DeDilectis, BSN, RN: Atria Hertlin Place & Massapequa Center Rehabilitation & Nursing

Raina DeDilectis, BSN, RN first visited Atria Hertlin Place in Ronkonkoma. Atria Hertlin Place, a welcoming independent living community, is nestled in the beautiful hamlet of Lake Ronkonkoma and set on 14 acres between Smithtown and Stony Brook. Here, Raina met with some staff and shared about Tully Law Group's services.

Raina also visited with the team at the Massapequa Center Rehabilitation & Nursing, a premier facility on Long Island. A family-owned and operated center, they pride themselves on caring for individuals with a custom-tailored plan of care with flexibility to address their specific variety of needs. Raina enjoyed speaking with the team about future collaborations.

Shanthy Hughes, LMSW: Our Lady of Consolation Nursing & Rehabilitative Care Center

Our next ECC, Shanthy Hughes, LMSW, visited Our Lady of Consolation Nursing & Rehabilitative Care Center. With 100 years of dedicated service, this facility is set in West Islip, overlooking the Great South Bay. While here, Shanthy met with Michelle in Case Management and Ginny from the Medicaid office.

Raina & Shanthy: Oyster Bay Manor Assisted Living

Together, Raina and Shanthy traveled to the Oyster Bay Manor Assisted Living. This family-owned and operated facility is in the heart of historic Oyster Bay on the scenic Gold Coast of Long Island. Oyster Bay Manor provides a community living experience that is comfortable and relaxed. Our ECCs met with Stephanie DePalma, Lauren-Marie Balletta, LMSW, and other Oyster Bay Manor team members here.

Michelle Morea, ECC: Gurwin Jewish - Fay J. Lindner Residences, Amber Court Assisted Living & Glengariff Rehabilitation and Healthcare Center

Our third and final ECC, Michelle Morea, ECC, visited three facilities this last week. First, she traveled to Gurwin Jewish - Fay J. Lindner Residences in Commack. At Gurwin Jewish, residents can enjoy new monthly activities to engage, entertain, and educate. Michelle chatted with the Gurwin team about the unique services Tully Law Group offers.

Then, Michelle went to the Amber Court Assisted Living in Westbury. World-class beaches, pristine parks, restaurants and shopping centers surround this facility. Their proximity to entertainment, recreational hubs, medical centers, and their amenities make it a desirable destination for seniors.

Michelle stopped by the Glengariff Rehabilitation and Healthcare Center to round off her visits. Glengariff is known for its small and intimate setting, which allows for a personalized brand of care. Michelle was thrilled to speak with the team members about Life Care Planning and its unique ability to benefit their residents.

In the realm of elder care and estate planning, the landscape can often seem complex and daunting. Families navigating these waters often seek guidance and support to ensure the well-being of their loved ones as they age. Enter Life Care Planning – a distinctive service offered by Tully Law Group, redefining the approach to elder care and estate planning on Long Island.

But What Exactly Is Life Care Planning?

Life Care Planning is a holistic approach to long term care that integrates elder law, estate planning, asset protection, care coordination, and advocacy to address the evolving needs of seniors and their families. It goes beyond traditional elder law by considering not only the legal aspects but also the practical and emotional dimensions of aging.

At Tully Law Group, we understand that every individual and family situation is unique. Life Care Planning begins with a comprehensive assessment of the client's current situation, including their health, finances, family dynamics, and long-term health care needs. Based on this assessment, a personalized plan is crafted to address immediate concerns and plan for the future.

On Long Island, where the aging population is growing, Life Care Planning fills a crucial gap in the continuum of care. Navigating the murky waters of Long Term Care Medicaid is daunting for any individual, and especially a caregiver who is already burdened with the stress of taking care of a loved one. Through our Life Care Planning, our experienced and knowledgeable Medicaid Attorneys will help you shelter your assets, remain in your home and apply for the Medicaid benefits you need to live a fulfilling life.

But What Sets Life Care Planning Apart from Other Elder Care and Estate Planning Services?

This plan includes not only necessary legal documents and Medicaid benefits to help pay for the care, but also advocacy and coordination assistance so the client can receive the best care possible.  Our team works closely with clients and their families to navigate the complex healthcare system, identify appropriate care options, and coordinate services to ensure that the client's needs are met. This can include assistance with accessing home care, coordinating with healthcare providers, supporting family caregivers or even transitioning to assisted living or nursing home care when necessary.

Moreover, Life Care Planning emphasizes collaboration and communication among all those involved in the client's care, including family members, healthcare providers, financial advisors, and other professionals. This interdisciplinary approach ensures that the client's needs are addressed comprehensively and efficiently.

At Tully Law Group, our team of experienced attorneys and Elder Care Coordinators are dedicated to providing compassionate and comprehensive support to seniors and their families. We understand the importance of preserving dignity, autonomy, and quality of life as individuals age, and we are committed to helping our clients and their families navigate the complexities of aging with confidence and peace of mind.

Life Care Planning is not just about planning for the future – it's about empowering seniors to live their best lives today. At Tully Law Group, we are proud to offer this unique service on Long Island, and we invite you to learn more about how Life Care Planning can benefit you and your loved ones. Contact us today to schedule a consultation and take the first step towards a secure and fulfilling future.

At Tully Law Group, we understand that the journey of aging often presents unexpected challenges, both for individuals and their families. Recently, we had the privilege of working with a couple facing significant health and cognitive issues, navigating a landscape of uncertainty and vulnerability. Their story underscores the vital role of advocacy and compassion in elder care.

This couple, living alone and grappling with daily tasks beyond their capacity, epitomized the fragility of aging without adequate support. For their children, witnessing their parents' decline was a harrowing experience, compounded by the realization of limited resources and assistance.

Enter Michelle Morea, our dedicated Elder Care Coordinator, whose keen insight and proactive approach proved instrumental in guiding this family through a complex web of challenges. Recognizing the urgency of the situation, Michelle swiftly mobilized resources to address the immediate needs of the couple and their concerned loved ones.

Navigating the intricacies of the Medicaid process and conducting thorough assessments, our firm ensured that the couple received the essential support they required. Initially granted a 24-hour live-in caregiver, it soon became apparent that this arrangement posed risks for one of the clients, particularly during nighttime hours.

Acknowledging the gravity of the situation, Michelle took decisive action, advocating tirelessly on behalf of the vulnerable client. Her unwavering commitment culminated in a successful appeal, securing 12-hour shifts to ensure round-the-clock care and safety for the client.

The impact of this outcome reverberates far beyond logistical arrangements—it represents a lifeline for a family grappling with uncertainty and fear. With newfound peace of mind, they can rest assured knowing that their loved one is receiving the attentive care they deserve, allowing them to remain safely in the comfort of their own home.

This story serves as a testament to the power of advocacy and empathy in elder care, illuminating the transformative potential of personalized support and proactive intervention. At Tully Law Group, we remain steadfast in our commitment to empowering families, one compassionate gesture at a time.
